We are finally open for business! Yes, we are happy about that too! A lot has changed in the last month for you, and for us. Your safety is our priority and although we have always been meticulous with our serialization procedures, the Utah Department of Health has asked us to be diligent in implementing the following procedures for all employees, patients, and visitors to our office. PERSONAL MASKS are REQUIRED- Patients and visitors are required to bring their own mask. Masks are to be worn in all public spaces and throughout your entire visit to our office. (obviously not while you are getting your teeth worked on)
INFORMED CONSENT FORM REQUIRED- Please contact our office and we will email you the informed consent form to sign electronically. Or, you may click this link COVID-19 Pandemic General Cons. Form After it is signed and your witness has signed it, take a picture of the form and email it to our office at firstname.lastname@example.org We need a signed informed consent for each patient before starting the appointment.
NO CONGREGATING in WAITING ROOMS – Our waiting rooms are no longer available. You will need to call or text our office at 801.610.1070 when you arrive and wait in your vehicle or outside the building until your room is ready. We are aware of the severe inconvenience this will be and we have adjusted our schedule to minimize this as much as possible.
LIMITS on VISITORS – Each patient is only allowed 1 individual to accompany them to their appointment. This will be especially hard on our large families. Please contact us and we will help you navigate this requirement.
PATIENT SCREENING – Each Patient and visitor will have their temperature taken and be asked a series of health and travel related questions to determine if it is safe for them to be in our office.
PRACTICE SOCIAL DISTANCING – Its the new buzz phrase and we should all be familiar with the 6 foot rule and the limits on touching surfaces to minimize the spread of the virus. The little ones will have the hardest time with this as we have had to remove our fun treasure tower from the waiting room.
